Written order checklist
  1. Patient’s full name and date of birth
  2. Order date
  3. Description of the item, with any options or accessories
  4. Quantity and, for supplies, frequency of use
  5. Prescriber’s name and National Provider Identifier
  6. Prescriber’s signature
  7. Supporting medical records when the patient’s plan requires them
